Download the application, and email the completed form to Roy Gerardi, or fax to 757-220-6113. You will be asked to provide a copy of your current lease, most recent paystubs, and verify a financial hardship due to COVID-19 (such as a lay off, reduced hours, furloughed, or COVID-related illness).
